This form must be completed after each event. The information you provide allows the DISC board to hold a record of the events hosted throughout the year, while also allowing your organization to grow by doing a little reflection.

When completed, this form will be shared with the DISC Executive Board. You may access the information at any time by simply asking the DISC Coordinator. If there is any confusion, the DISC Coordinator or the Post Event Financial Officer will reach out for clarification.

  • Don't use the organization name's abbreviation.
  • Date Format: MM slash DD slash YYYY
  • We would like to know if the money allotted for the event needs to be adjusted in the future.
  • Let us know how the event went. What worked well? What needs to be improved? Anything the DISC Board needs to discuss?
  • How did you all publicize the event?
  • Please upload a link to at least 3-5 photos from your event. Or, upload the photos directly to the Post Event Form here. Please include names and class year of the people included in the photos if possible.
